Brazilian Butt Lift: The Only Solution for a Curvaceous Posterior?

What is a Brazilian Butt Lift?

What is a Brazilian Butt Lift (BBL)? A BBL, also known as a Brazilian butt lift, is a fat grafting technique used to augment the buttocks, employing massive volumes of fat. Generally, 1 to 3 liters of fat are used in each buttock during a Brazilian butt lift. These large volumes aim to significantly enlarge the buttocks. A Brazilian butt lift is only possible if two conditions are met: the patient must have sufficient fat to aspirate, and the buttocks must have sufficiently lax skin to accommodate this large volume of fat.

First, the patient needs enough body fat to provide the necessary volume for buttock augmentation. This doesn't mean the woman has to be obese, or become obese! It's important to be clear on this point. There are adequate ratios of body fat to muscle mass that allow for BBL surgery without the patient being obese.

Once liposuction is performed, this fat will be processed. After processing, it will be ready for buttock augmentation. We know that fat, when placed in an area without "space" to receive it, suffers and dies. One of the factors that can lead to the loss of this grafted fat in the "buttocks" is a lack of space. This means that the buttock area, including its skin, must be relatively "lax" to accommodate the fat volumes.

Brazilian Butt Lift: A Controversial Procedure?

The number of complications and deaths related to this cosmetic surgery appears to have increased. According to the Australasian Society of Aesthetic Plastic Surgeons (ASAPS), the mortality rate for BBL is approximately one in 3000 patients. This makes it potentially the most dangerous surgical procedure. Buttocks are becoming the new breasts. And because two are better than one: cosmetic surgery often involves taking fat from one area of the body and transplanting it to another (but the issue of fat banks is a whole other story).

BBL price: However, the price of a BBL doesn't seem to be a deterrent; the procedure is increasingly popular (it's the second most performed). Regarding the mortality rate, a controversy arises: is it the fault of the surgery, which is not yet safe enough, or the fault of those who want it at all costs? Measures must be taken to prevent further tragedies. Patient safety is the top priority for plastic surgeons, and this collaboration will be important and unprecedented. What causes the death of one patient out of 3,000? The main reason is embolism due to small fragments of fat entering the bloodstream.

Post-Operative Care

Complete rest is prescribed for the 48 hours following a BBL operation. It is also recommended to sleep on your back for about two weeks, and it is possible to sit from the second day after the BBL operation. From the third day, a quiet life can be resumed, avoiding strenuous activities, saunas, Turkish baths, and sun exposure. During the first two days after the operation, swelling, bruising, and pain may appear in the treated area. After 7 to 10 days, professional activity, if not excessively strenuous, can be resumed. It is also recommended to wear a compression sleeve continuously for about a month.

With the latest fat processing techniques, the percentage of reabsorption of the transplanted adipose tissue will be very low. However, it is necessary to assume an increase in volume on each side of at least 250-350 cc of adipose tissue, which will therefore not be a viable option for already very thin people.

Alternatives to the Brazilian Butt Lift?

There are essentially two alternatives to a Brazilian butt lift:

  • Buttock augmentation with implants: Although the amount of autologous material (taken from the thighs, hips and/or abdomen) does not cause allergies and guarantees a completely natural result, it may not be sufficient to reconstruct hollow areas. This is particularly the case for thin individuals who do not have a sufficient amount of body fat. In the latter case, the option of implanting prostheses will be pursued, provided that the individuals have elastic and firm skin and a good level of muscle tone. The procedure allows for the implantation of a volume of approximately 400 cc per buttock. The intervention consists of making an incision of approximately 5/6 cm in the intergluteal sulcus, so that the scar remains hidden between the buttocks. In this space, the prosthesis will be inserted, which generally has a smooth surface and is made of the latest generation silicone. It is also practical to use drains that will only be left in place for a few days;
  • Buttock augmentation with hyaluronic acid: Buttock reshaping can also be performed using hyaluronic acid in 10 ml ampoules of hyacorp injected by a cannula deep into the tissues. In this case, the surgeon operates under local anesthesia and no hospitalization is necessary. At least 3-4 ampoules per side will be needed to achieve sufficient visible reshaping;

The duration of each procedure may vary depending on the procedure applied and the patient's condition. In general, complications related to the intervention are infections (to be prevented by antibiotic treatment), hematomas, asymmetries, and irregularities in the skin profile.
