Deciding Between Breast Implants and Fat Grafting?

What are breast implants?

Breasts are a crucial part of a woman's body, symbolizing femininity and sensuality. Breast augmentation with prostheses is a surgical procedure that increases breast size, corrects shape, and improves appearance using breast implants, primarily made of cohesive silicone gel today.

Breast augmentation with prostheses, involving the implantation of the latest-generation breast implants, remains the most frequent cosmetic surgery procedure. It significantly impacts patients' psycho-physical well-being, even before the aesthetic results are visible.

What is fat grafting?

Fat grafting for breast augmentation involves extracting fat from one area of the body and injecting it into the breasts. Because it uses the patient's own cells, it avoids allergic reactions and ensures a natural result. After selecting the fat donor site, the surgeon harvests fat cells, which are then processed before injection.

After purification, the fat is injected into the breast using small cannulas, leaving tiny scars in the inframammary fold (1-2 millimeters) that quickly disappear.

Breast augmentation by fat injection offers several advantages:

  • Very fast post-operative recovery (within 24 hours).
  • No risk of allergies (your own fat).
  • Removes excess fat from other areas.
  • No foreign body implanted.
  • Extremely natural final appearance.

However, this technique has limitations:

  • Not suitable for very thin women (requires significant fat removal).
  • Only increases breast size by one cup size (infeasible for those starting with a size zero).
  • Unpredictable results (depends on fat cell absorption).
  • May require repeat procedures.

Choosing between breast implants and fat grafting

Fat grafting is ideal for those who want breast augmentation and to remove excess fat from the abdomen, hips, thighs, and knees (body contouring).

Breast implants are better for those without significant localized fat deposits. Implants can increase breast size by more than two cup sizes, unlike fat grafting (typically one cup size or slightly more).

With implants, the result is immediate and predictable. Fat grafting often involves partial and natural fat reabsorption, which increases with the amount of injected fat. A second procedure after 3-4 months is often needed for a stable result. Smaller, teardrop-shaped implants result in a more natural look.

After lipofilling, keep the breasts free from pressure for about a month to improve fat absorption. After implant surgery, wear a post-operative bra for about four weeks and avoid strenuous activity. A compression garment should be worn at the fat harvesting site after fat grafting.

Even with the latest cohesive silicone gel implants (which remain contained in case of rupture), they do not have an indefinite lifespan and should be replaced after 15-20 years.

Fat grafting results may decrease with weight loss, unlike implants. Both are safe when performed in a clinic.

Lipofilling is done in a clinic as an outpatient procedure under sedation. Breast augmentation with implants requires general anesthesia and a one-night hospital stay.

Lipofilling requires no breast incisions (fat is injected via fine needles). Small incisions are made at the liposuction sites. Implant surgery incisions are hidden, and stitches dissolve on their own.

Protect incisions from the sun for 6-7 months.

Both procedures involve a few days of antibiotics and anti-inflammatories, and a one-week check-up.
