What are All-on-Four and All-on-Six?

These two implant techniques, "All-on-Four" and "All-on-Six," allow for the rehabilitation of entire dental arches, both mandibular and maxillary, in just 24 hours.

To easily understand what an All-on-Four or All-on-Six implant is, imagine being able to enter a dental office with a complete denture or a few teeth and leaving with 12-14 teeth per arch, in your desired color and shape.

Both "All-on-Four" and "All-on-Six" implantology techniques achieve this result, but they differ in one key aspect:

  • All-on-Four implantology, as the name suggests, involves rehabilitating an arch by inserting 4 titanium implants;
  • The All-on-Six technique uses six implants.

All-on-Four and All-on-Six and Immediate Loading

Implantology allows us to replace missing teeth with artificial roots inserted into the bone, and porcelain crowns that emerge from the gum, screwed onto these roots.

Characteristics of All-on-Four and All-on-Six

All-on-Four

The "All-on-Four" implantology technique may suit you if you already have a denture that rests on a few residual teeth that don't allow you to eat and/or speak properly. This method is also often used in patients with weak maxillary and mandibular residual bone.

In fact, the key characteristic of an "All-on-Four" prosthesis is having 4 implants positioned in the bone, but with the two outermost ones angled at 45 degrees, so as to bypass important anatomical areas that should not be touched during surgery.

This technique with an All-on-Four implant is an ideal solution if you want to have fixed teeth quickly, without painful procedures, and can also be applied if you have been edentulous for a long time.

People who have been edentulous for a long time have very large maxillary nerves or sinuses, and by placing these angled implants, we can preserve them.

Furthermore, the "All-on-Four" implantology technique allows us to avoid unpleasant and lengthy bone grafting procedures, resulting in longer healing times.

 

All-on-Six

A variation of the "All-on-Four" implantology technique is "All-on-Six," which has particular characteristics that distinguish it from "All-on-Four," while other characteristics are similar.

In cases where the amount of bone allows, we can insert a greater number of implants, namely 6.

Even with the All-on-Six technique, we can have fixed teeth in 24 hours, provided that the quality of our mandibular or maxillary bone is good. The advantage of the All-on-Six technique is the great aesthetic result: with straight implants, it is much easier to obtain an aesthetic result almost identical to that of a mouth with natural teeth. In fact, the use of artificial gums can often be avoided.

 

All-on-Four and All-on-Six Implantology: Advantages

The reasons why it is preferable, when possible, to use the All-on-Four and All-on-Six techniques are numerous. Here they are:

  • Eliminates the waiting time for osteo-regeneration;
  • Considerable time saving: it is possible to complete the work in a single session;

The All-on-Four system is the ideal solution for patients who have worn complete dentures for many years and in whom significant bone loss has occurred over time. Today, All-on-Four is the most commonly recommended solution for a dental implant. The four implants offer sufficient stability and strength to support a lifelong fixed dental bridge. The All-on-Six method is the best option from a functional and aesthetic point of view, with an excellent long-term prediction. It is always preferable to have as many prosthesis bearers as possible. This method is therefore an excellent solution for edentulous jaws. But it must be borne in mind that in some situations, due to the low quantity or poor quality of the bone, the number of implants that can be placed is limited.
